Hashimoto’s, Gluten, and The Immune System – Things Every Patient Should Know

What does it really take to be gluten free?

This is something that I discuss with patients almost every single day in my clinic and it is surprising the misconceptions that many people have about gluten free diets.

Many thyroid patients have discovered, from various sources, that the first thing they should do after getting diagnosed with Hashimoto’s Disease, an autoimmune form of hypothyroidism, is implement a completely gluten free diet.

When you talk to people that have made this discovery and have made this dietary change on their own these are the things that you will hear them say:

“I am kind of gluten free”

“I am almost gluten free”

Here is the problem:

If you have an immune sensitivity to gluten, a protein found in barley, wheat, rye, spelt, kamut, and processed oats, then your body is creating antibodies to that food every single time it sees it. That means you are creating inflammation in the stomach and throughout the entire body.

So even if you go two days, or even four days gluten-free, and then you have just a little bit, well, you’ve completely reversed everything that you’ve done, and you’ve basically gone back to square one.

Every time you eat it you elevate the antibodies and inflammation all over again and it take weeks or months to clear this out of the body. ‘

What happens for some patients is they get frustrated because they go a few days, they eat a little bit, they go a few days, they eat a little bit, and they never actually make any progress, and so they’ll finally just say, “The gluten-free diet is not working.”

The reality is that for some Hashimoto’s patients, by just removing gluten they make a lot of progress without doing a lot of the other stuff that can be done to really modulate the immune system in Hashimoto’s patients.

For 99 percent of Hashimoto’s patients this dietary step is the first step that must be taken, and that’s a step that you can do all on your own, not knowing anything else about your problem or how the immune system has malfunctioned.

Once a Hashimoto’s patient has made initial dietary changes then diagnostic steps can be taken to determine all the other immune triggers and pathways that all not functioning properly.

What sort of other things does a Hashimoto’s patient need to uncover to get on the fast track to recovery?

What are the every sources of inflammation that you are unaware of but could eliminate if discovered?

What is the extent of the damage to the GI tract from the inflammation?

What is the ORDER of steps that must be taken to move from point A to point B?

The traditional model simply looks at some labs and concludes, “these numbers are off, let’s give you some Synthroid and let those numbers come back into the normal range.”

A lot of the time, on paper, the lab ranges are going to come back into the normal range, but that doesn’t mean that you feel and function good. If those numbers look good and you feel horrible, what have you accomplished?

In all reality, you haven’t accomplished much. They are just manipulating numbers on paper.

Not that you don’t need that part of the treatment, but obviously it’s not addressing the real issue.

So just to recap, if you’ve started a gluten-free diet, it really needs to be 100 percent gluten-free diet, or it’s not a gluten-free diet at all, and you’re not going to get any benefit from it.

Two, if you’ve had this problem for a long time and you’re just making that change, it may take a while before you see any benefit from it. There’s the possibility that you may start to see changes within the first week, but for other people there’s so much damage done as a result that it could take months, and you just have to know that that it is something that has to be done.

What Is Lipoma?

We are told that lipomas appear just because of a genetic predisposition. I have a hard time believing that. I feel that something external triggers the genes to produce excessive amounts of fat cells. I have a hypothesis as to why this happens.

Basically, I see only one reason for the growth of a lipoma. It has to be a triggering effect. And I think about the external or environmental triggers. It makes no sense that the body – developed for ages – would create harmless lumps without giving any reasons to create them.

The evolution has a habit of removing all features that are not useful. I mean, we all still have our tailbone, but it has decreased dramatically. Our ancestors had a tail, but because modern man no longer needs it, it has almost disappeared.

In the same way, I think all the tumors would have disappeared long ago, if they are not somehow beneficial for us.

So what then is a lipoma?

Dr. Simoncini who is a controversial Italian oncologist believes that lipoma is a cyst caused by fungi. He believes that fungal infection produces a chain reaction to which the body responds by packetizing the fungal cells with fatty tissue.

I’ve since read expanding my view that it is not necessarily fungus that causes the inflammation, but perhaps a pleomorphic bacteria. Pleomorphic means that either all microorganisms share an ability to resemble each other, or that some bacteria are able to transform themselves in us as viruses, bacteria or fungi.

There are many other clues that point to the bacterial causation. Many known carcinogens (eg, radiation, dioxin, tobacco, asbestos, red meat, alcohol, the immune-suppressing drugs, heavy metals) are praised by bacteria and fungi.

What are the causes lipomatosis?

I think lipomatosis happens because somehow escaped microorganisms or infected cells from the lump create new inflammation locations. I do not think there is any predisposition or dislocation in our genes, if we except the translocation is either inherited by one of our birth parents. In this case, the mutation caused by the microorganism was somehow distributed to the germ cells.

While this is all just speculation, it gives strong evidence, that properties can be found in the course of our lives and our children. This is known as epigenetics which means that non-genetic factors can result in the organism genes behave differently. The DNA that you develop during lifetime makes your children already prone to lipomas. In epigenetics, there is not necessarily mutations but a certain kind of cell memory.

But even if inherited, I do not think lipomas would begin to appear, without a reason. Perhaps some of us have genes that have already taken in a high-alert mode, and even if there is no need to start with the body fat cells gathering around the inflammation, they do so.

But what follows is that even if you do have an inherited translocation, how to change the situation, how your genes by helping to eliminate the cause of the inflammation and thus switching off the genes that produce the clumps are expressed.

Ergonomics improve the efficiency of the workplace by prioritising the consideration of usability, comfort and employee safety, and incorporating such in the design – or redesign – of the working environment. The New Cardiopulmonary Resuscitation Guidelines

Cardiopulmonary resuscitation (CPR) is a lifesaving technique useful in many emergencies, including heart attack or near drowning. A situation when someone’s breathing or heartbeat has stopped. CPR can keep oxygenated blood flowing to the brain and other vital organs until more definitive medical treatment can restore a normal heart rhythm. Ideally, CPR involves two elements: chest compressions combined with mouth-to-mouth rescue breathing. However, what you as a bystander, should do in an emergency situation really depends on your knowledge and comfort level. The bottom line is that it’s far better to do something than to do nothing at all.

This is what I, as a physician of thirty years, recommend to those who ask me what they should do.

If you’re not trained in CPR, then provide hands-only CPR. That means uninterrupted chest compressions of about 100 a minute until paramedics arrive. You don’t need to perform rescue breathing. If you’ve previously received CPR training, but you’re not confident in your abilities, then just do chest compressions at a rate of about 100 a minute. This is the “magic number”, 100! It’s the only real number you need to remember. If you’re trained, and confident in your ability, then you can opt for one of two approaches: Alternate between 30 chest compressions and two rescue breaths, or just do chest compressions.

This advice applies only to adults needing CPR, not to children.

Cardio Pulmonary Resuscitation changed this year, as statistically, “mouth to mouth” shows no significant improvement in CPR effectiveness. This applies to adults and not to children. Mayo Clinic findings have convinced experts to remove the mouth to mouth component out of the protocol traditionally endorsed by the American Heart Association and The American Red Cross. Even without mouth to mouth, the compressions alone, are delivering about 70% oxygenated blood to the brain, giving the victim a huge chance for survival. The alternative, “doing nothing at all”, is fatal.

I am often asked, why doesn’t this apply to children? The reason is that most CPR events occur with older adults who have had a cardiac event which renders them unconscious and unresponsive. Children, on the other hand, are generally healthy and robust from a cardiac stand point. They do not generally have heart attacks, and do not usually suffer from cardiac arrest unless already air starved from a life threatening pulmonary condition such as an asthma attack, depleting them of oxygen. Therefore, replacing oxygen (mouth-to-mouth breathing) is critical for their survival.

Do the smart thing. Get certified in CPR. Always be prepared to make a difference, and be able to save someone’s life.

Why Do Nurses Wear Uniforms?

When I go to a hospital with family members I really can’t tell who’s a nurse anymore.

My mother was taken to the ER just this week and while we sat and watched the general busy-ness and absolute professionalism of the ER staff, the only people I could really identify were the doctors and they weren’t even wearing a uniform!

In fact we mistakenly asked a cleaner for a pan because she was wearing a uniform that looked more professional than the nurses who were all wearing a mishmash of scrubs and gowns; one nurse had a particularly fun jacket with smiley-faced vegetables all over it. Cute but professional? Hmmmm.

As for scrubs, they might be comfortable but they always look so dowdy and overwashed; the practise of wearing scrubs seems to me to be a case of “life imitating art” or in this case TV! The general unidentifiable jumble of uniforms in the ER detracted from the consummate nursing skill that was all around us.

So I have to suggest that if nurses want to wear a uniform then maybe we should all be wearing the same easily identifiable one – after all that is the whole point of a uniform. Uniforms stand for conformity and consistency. Uniforms are meant to allow people to instantly recognise a cohesive group of important individuals.

If we roll back time as Rich suggests then each level of nurse would be easily recognisable, just as we were when I started nursing in 1975. Up until fairly recently there wasn’t much confusion at all about who was who. Registered nurses wore white shoes and uniforms, student nurses wore caps, blue uniforms and brown shoes, EN’s wore their own colours and so on! Personally I’m not keen on any of that and felt then as I do now that a uniform can be a barrier.

A uniform may identify us but it can also set us apart because uniforms can confer some level of authority and exclusivity that might interfere in the nurse-patient relationship. I believe that type of hierarchical exclusivity was one reason why nurse’s uniforms became the hodgepodge, egalitarian affair they have become. The general assortment of uniforms was meant to place us on a more professional and level field as it were, thus nurses’ uniforms became so casual and diverse that in the 21st century a highly qualified and professional nurse wearing a coat with smiley-faced vegetables seems perfectly okay.

So why do we wear uniforms? Nursing hasn’t always had the professional status we have now – uniforms actually reflect our roots in the lower-class service industries of the 19th century.

That’s the reason for the aprons and caps and big buckles that nurses used to be rigged out in up until the 1960′s and 70′s. Florence Nightingale didn’t do us any favours either by placing us firmly in the armed services and subservient to medical consultants; thus the uniform of a maid or handmaiden. Please….let’s not go back there!

I haven’t worn a uniform for 20 years and I haven’t had any problems with being recognised as a nurse – but that’s probably because I work in the community. Hospital-based nursing does need some different consideration due to the numbers of health professionals working there. However, in the community setting, my nursing practise is recognised by my clients through the way I care for them not by what I wear; and that’s how I knew who the nurses were in the ER this week. They were the ones that spoke gently to my mum, helped her respectfully and effectively and explained what the doctor just said.

I observed what I like to term as the “patient-nurse attunement”. This particular nursing quality just doesn’t exist between other health professionals and their patients. Our nurse attunement connects us to the basic needs in a human — that is to build a relationship, no matter how short term. This nurse-patient attunement allows us to gently and respectfully care for our patients physically, emotionally and socially. I think this is nursing’s absolute speciality and that is how we are known.

By all means have a uniform but let’s have one that is consistent — states what we do and doesn’t allow a barrier to come between us and our patients. But above all don’t believe for a minute that the uniform is what defines you as a nurse.

Tips to Choosing an Electric Mobility Scooter

So you’re thinking about buying an electric mobility scooter to increase your independence and mobility – a big decision and a sizable investment, and since there are quite a few available on the market today, a little research can help ensure that you get the scooter that is best suited to your needs and your pocket.

Budget. The first thing you need to decide is how much you can afford to spend on your scooter. The price range for powered scooters is huge – anywhere between $500 or $600 to $8000 or $9000 or more.

Weight. The next most important decision is the carrying capacity required of the scooter. Do you need a bariatric scooter? Bariatric electric scooters have a weight capacity of 400 lbs which would include the weight of your personal items. Other scooters have a maximum weight capacity of up to 250 lbs or 300 lbs including your personal items.

Purpose. Ask yourself what you want to be able to do with your scooter. Do you need to use it every time you leave your home or just on the longer outings? Do you just want to drive around in the mall or supermarket, or do you want to be able to tackle sidewalks, outdoor recreational areas and anywhere else that takes your fancy.

Portability. Do you need a scooter that is designed to be portable as well, one that can be easily broken down into a few components that will fit inside the trunk of your car, and even be transported as luggage on planes, trains and coaches.

Cruising Range. Is the distance that you can go on one charge important to you or do you just need something for very local errands? Some scooters can only do 6 or 7 miles on a single charge, whilst others can do 15 or 20 miles.

Turning Radius. Is the turning radius of the scooter an important factor for you. Turning radius varies significantly, and can be anywhere from 32″ to 64″ on most powered scooters. So think about where you want to use your scooter – supermarket aisles tend to be quite wide, but the corner store is not. Some restaurants and shops can be quite tight too.

Storage. How much stuff do you need to carry around with you on your scooter? They all have a basket that can accommodate a few items, some have storage under the seat. Bear in mind that you can purchase additional storage baskets that attach to the seat back or various nylon bags that attach either to the seat back or armrest. And if you need to travel with oxygen, you can get a basket specially designed for that too.

Wheels. Powered mobility scooters come with either 3 or 4 wheels – some solid and some pneumatic, so you do need to think about the practicality of repairing flat tires and punctures if you are leaning towards pneumatic tires. Generally speaking, 4-wheeled scooters have a wider turning radius but are more stable. 3-wheeled scooters do have anti tip safety features in place to improve their stability.

The Three Main Styles of Tonneau Covers

If improved aerodynamics aside from style and safety is what you want for your truck, then installing tonneau covers is the way to go. This truck accessory comes in various styles.

The different styles are each designed to transform your truck and make it work better. You can choose among three main styles – hard, soft and retractable.

Soft – such style would consist typical of a soft top cover or tarp supported by a framework. The tarp is usually made of vinyl or leather, which can be pulled over from an aluminum-made framework.
